Hover on Games, click on Import Export Questions, You will find a link to download a sample CSV under the Import tab.

How do I set number of attempts per question and total questions to a game?

Hover on Games, click on All Games. Select a game and you will find options to set total questions and number of attempts per question in a game.

How do I quick-add new question to a game?

Select a game from All Games and you will find a section to add new question under the Game settings section. You can quickly add a new question and save the game.
